CASE MANAGEMENT

is provided by social workers who do assessments in clients' homes to evaluate their needs, arrange for services to meet those needs, and monitor their situations.

FRIENDLY VISITORS

make regular visits to lonely and isolated individuals who need someone to care and be supportive. An intergenerational outreach includes high school and elementary school students who send cards to seniors to celebrate their birthdays.

INTERGENERATIONAL PROGRAMS

link senior clients with young people of school age in pen-pal arrangements.

REFERRAL TO IMPORTANT LOCAL RESOURCES

Experienced social workers with a long term community presence identify valuable support services for homebound clients.

TELEPHONE REASSURANCE PROGRAM

provides peace of mind to those living alone and their families. Telephone calls at specified times assure that individuals are safe and well. For some clients these calls may be the only outside contacts they have.

SHOPPING SERVICE

volunteers help the homebound and disabled with grocery and pharmacy shopping.

TRANSPORTATION/ESCORT SERVICES

are furnished to individuals in need of transportation to doctors or medical centers.

BLOOD PRESSURE TESTING & BLOOD SUGAR SCREENING

are provided by registered nurses once a month on Saturday mornings.
